چکیده

Cover illustration " Weighing of the Heart " from chapter 125 of " the Book of the Dead " , a famous papyrus from the 19th Dynasty of the New Kingdom in ancient Egypt, dated around 1250 BC. Second thoughts Tage Danielsson from " Thoughts from the root " (1974) I believe in doubt. Doubt is the basis of all knowledge and the engine of all change. Doubt is, to make doubly sure, the prerequisite of faith. He who believes without first doubting is an exulting blockhead and a tinkling cymbal. And he who believes without at the same time doubting is hardly less of an exulting blockhead and hardly less of a tinkling cymbal. Faith can remove mountains but doubt can put them back in place again. Reprinted with permission from the copyright holder I wish to dedicate this book to my family. Abstract 4 Abstract Background: Myocardial failure and dysfunction is not uncommon during critical illness and following cardiac surgery. For optimal treatment, a better understanding of the effects of inotropic drugs is needed. In this thesis, two non-adrenergic mediated inotropes, milrinone and levosimendan were studied in different models of myocardial dysfunction. The study aims were to assess the following: the effects of milrinone on blood flow in coronary artery bypass grafts during CABG surgery; the effects of milrinone on left ventricular diastolic function during post-ischaemic myocardial dysfunction; whether milrinone or levosimendan are protective or injurious during acute myocardial ischaemia, and if levosimendan potentiates myocardial function when added to milrinone in an experimental model of post-ischaemic (stunned) myocardium. Material and Methods: In Study I, 44 patients undergoing coronary artery bypass surgery (CABG) were included as subjects. Milrinone or saline was administered in a single dose during cardio-pulmonary bypass (CPB) and coronary graft flow measurements were recorded after 10 and 30 min following CPB. In Study II; 24 patients undergoing CABG had estimations of peak ventricular filling rates made before and after CPB with administration of milrinone or saline in a single dose during CPB, performed by assessment of the rate of change in diastolic cross-sectional left ventricular area. In Study III, energy-metabolic effects of milrinone and levosimendan were measured in an anaesthetized porcine model during 45 minutes of regional myocardial ischemia. Microdialysis sampling of metabolites of local ischemic metabolism allowed assessment of glycolytic activity and the degree of myocardial calcium overload. In Study IV, in a porcine model …

Mechanism of Action of the Thyroid Hormone on the Heart

SUMMARY  The foliowing cardiac effects may be attributed to thyroxin:  1-Thyroxin augments all anaerobic processes in the body includ::ng the heart, and decreases the glycogen content of the heart ( 1, 2, 5, 27). The resistance of the heart to anoxia is increased in hyperthyroidism ( 25). 2- Thyroxin influences the cardiac weight and prevents cardiac atrophy (3, 8, 10, 11, 19, 20, 21, 30...

Cardiac Non-myocyte Cells Show Enhanced Pharmacological Function Suggestive of Contractile Maturity in Stem Cell Derived Cardiomyocyte Microtissues

The immature phenotype of stem cell derived cardiomyocytes is a significant barrier to their use in translational medicine and pre-clinical in vitro drug toxicity and pharmacological analysis. Here we have assessed the contribution of non-myocyte cells on the contractile function of co-cultured human embryonic stem cell derived cardiomyocytes (hESC-CMs) in spheroid microtissue format. Microtiss...

Non-thyroidal illness syndrome and cardiopulmonary bypass in children with congenital heart disease

Abstract Background: The thyroid hormones influence on all metabolic pathways. After heart surgery using cardiopulmonary bypass (CPB), serum T3 decreases and remains low for at least 24 hours. Several studies on pediatric have reported reduction of thyroid hormones after heart surgery. This study aimed to investigate the status of thyroid function tests in children with CPB surgery. Metho...

Nitrous oxide administration and hemodynamics.

EDITORIALS 211 inotropic agents (such as the catecholamines, calcium ions, and digitalis), and paired electrical stimulation. Entropy also increases during congestive heart failure, which may be thought of as an expression of a defect in the utilization of energy, a partial chemicomechanical uncoupling. The entropy of the myocardial cell tends to be reduced by negative inotropic agents, such as...

Newer treatments for decompensated heart failure: focus on levosimendan

Acute heart failure (AHF) is a major cause of hospitalizations. Severe dyspnea, pulmonary congestion and low cardiac output with peripheral vasoconstriction and renal hypoperfusion is a main form of clinical presentation. Most patients with acute worsening have a pre-existing decompensated chronic heart failure (ADCHF), but AHF may also occur as a first manifestation of a previously unknown hea...

Effects of two-months Nigella sativa supplementation on cardiac hemodynamics and adrenergic responsiveness.

OBJECTIVE To study the effects of two, months Nigella sativa (N. sativa) oral supplementation to normal rats on cardiac haemodynamics in vivo, the ionotropic and chronotropic properties of the isolated hearts in vitro, and the cardiac responsiveness to progressive adrenergic stimulation by isoproterenol. METHODS The cardiac workload, after 2 months of N. sativa oral supplementation to normal ...
